Needles or Cannulas? Choosing Your Delivery Method for Filler and Mesotherapy

Ask ten practitioners whether they prefer needles or cannulas and you will get a spread of answers, most of them reasonable. The honest position is that they are different tools rather than competing ones, and most well-stocked treatment rooms keep both. What follows is how they differ mechanically, and what we stock in each, so you can match the tool to the treatment rather than to habit.

The mechanical difference

A sharp needle has a bevelled tip that pierces tissue directly. That gives you precision and a predictable path, which is exactly what you want for superficial, detailed work where placement matters more than distance travelled.

A cannula has a blunt tip and a side port. Rather than piercing tissue, it slides through it, which is why cannulas are associated with less trauma to the hypodermal layer. You enter once through a pilot hole and then work outwards from that single entry point.

What we stock: needles

BD Microlance 3 needles come in a broad range of gauges and lengths, with a sharp bevel for easy penetration and smooth advancement, reduced friction through tissue, and a colour-coded hub so you can identify the gauge at a glance. They are sterile, single use, and supplied 100 to a box in both regular wall and thin wall options.

The range runs from 16G down to 30G in lengths from 1/2" to 2", so you can go from drawing up and reconstitution at the thicker end through to fine superficial work at 30G. The colour coding is more useful than it sounds when you are working at pace with several gauges on the tray.

For very fine, shallow delivery there is also the BD insulin syringe with an integrated 30G x 8mm needle, a 0.3ml unit with a bevelled needle, latex-free piston and 100 per pack. The integrated design removes hub dead space, which matters when you are working with small volumes.

What we stock: cannulas

JBP Nano Ultra-Thin Wall Cannulas are a blunt-tip design intended to minimise damage to hypodermal tissue, helping reduce the pain and swelling that can occur during injection. They are made from stainless steel with an ultra-thin wall, which supports efficient product flow for a given gauge, and they are sterile and single use with 24 individually packed pieces per box.

Two sizes are available, 18G 70mm and 21G 50mm. The longer 18G suits broader areas where you want good reach from one entry point, while the 21G 50mm is the more manoeuvrable option. The listing notes they may be used for compatible aesthetic procedures involving products such as dermal fillers or mesotherapy solutions, subject to the product instructions, the cannula specification and your own assessment.

How to think about choosing

Start with the treatment plan rather than the tool. Fine, superficial and highly detailed placement tends to favour a sharp needle, where the precision of a direct entry is the point. Broader areas covered from a single entry, or work where minimising tissue trauma is a priority, is where the blunt-tip cannula earns its place.

Then check compatibility, which is the step most easily skipped. Gauge has to suit the viscosity of what you are delivering, since a thick Sub-Q gel and a thin mesotherapy solution do not behave the same way through the same bore. The ultra-thin wall on the JBP cannulas helps here by giving more internal diameter for a given gauge. Length has to suit the area and the reach you need from your entry point, and the product's own instructions for use may specify or restrict the delivery method, so those take precedence over general preference.

Practical points worth remembering: cannulas usually need a pilot hole made with a needle of an appropriate gauge, so you will be using both in one treatment. Neither is sterile once the individual packaging is compromised, so do not use a damaged pack, and never reuse either. And whichever you choose, everything goes into a proper sharps container and is disposed of in line with your local clinical waste regulations.
